Module 2 Data Entry Form - The Basics
eLogic Genesis offers the Module 2 Data Entry Form in an electronic version. The Module 2 is used for Community Action Agencies to document financial information that is not included in typical financial accounting. State CSBG Lead Agencies fill out and submit the Module 2 Data Entry Form annually to the Office of Community Services (OCS).
Some Community Action Agencies have specific data entry exceptions where agencies are not required to fill out specific information. Please review these rules with your state administrator prior to filling in the Module 2.
IMPORTANT - Before you get started...
The Module 2 Data Entry Form has a few distinct features that administrators should be familiar with prior to filling it in. Please read the items below carefully.
Static Form
Note that the Module 2 Data Entry Form does not show changes over time. This is an electronic form that will overwrite the previous version with every save. Please make sure that any information saved is relevant to your reporting period.
Manual Save Required
The Module 2 Data Entry Form does not save automatically; users must click Save CAA Form Report after making any changes to update the form. The Save CAA Form Report button appears at both the top and bottom of the page.
Assign fNPI/SRV Codes and Costs to Services
All CSBG related services must have appropriate NPIs assigned to them in order to set a cost/value for all CSBG services offered. Instructions here:

Each field on the Module 2 Data Entry Form has a short description. Some fields have additional information in the sections below. Please use these resources to assist when filling in and saving your agency's Module 2 information.

Section A: CSBG Expenditures by CSBG Eligible Entity
Amounts for rows A.2a-A.2f are referenced by the SRV/NPI codes that have been assigned to your agency's services. The total costs of these services will calculate and display in the report results. Once all relevant services have been assigned an SRV/NPI code and have a cost entered (either by default service cost or manually entered cost), no further action needs to be taken for these items.
Below are the Expenditure Domain CSBG Funds and how they correspond to the SRV/NPI codes
- A.2a. Employment = Employment Services (SRV 1)
- A.2b. Education and Cognitive Development = Education and Cognitive Development Services (SRV 2)
- A.2c. Income, Infrastructure, and Asset Building = Income and Asset Building Services (SRV 3)
- A.2d. Housing = Housing Services (SRV 4)
- A.2e. Health and Social/Behavioral Development (includes nutrition) = Health and Social/Behavioral Development Services (SRV 5)
- A.2f. Civic Engagement and Community Involvement = Civic Engagement and Community Involvement Services (SRV 6a-f)
- A.2g. Services Supporting Multiple Domains = Services Supporting Multiple Domains (SRV 7)
A.2. CSBG Expenditures Domains - This area of the Community Action Agency Form covers all agency expenditures related to:
- A.2h - Linkages
- A.2i. Agency Capacity Building
- A.2j. Other
A.3. CSBG Expenditures Domains - Of the CSBG funds reported in A.2., report the total amount used for Administrative costs. For more information on what qualifies as Administration costs, refer to IM37. Typically, CAAs should only allocate a small percentage of their CSBG funding for administrative costs, usually between 10%-20%.
A.4. Details on Agency Capacity Building Activities Funded by CSBG - Identify which activities were funded by CSBG under Agency Capacity in Table A.2 by checking all that apply.
Section B: CSBG Eligible Entity Capacity Building
B.2. Hours of Agency Capacity Building (e.g. training, planning, assessment) - Enter all combined hours related to capacity building for all CSBG Eligible Entities staff members.
B.3. Volunteer Hours of Agency Capacity Building (e.g. program support, service delivery, fundraising) - Enter all hours related to capacity building related to volunteers in this section. If agencies are interested they can create volunteer accounts with eLogic Genesis and track and tag volunteer hours in the Note Section. This is useful for both tracking volunteer hours as well as being able to report on which volunteers were low-income both of which are required for this section.
Section C: Allocated Resources per CSBG Eligible Entity
C.2. Allocated CSBG - This is the total number of dollars the agency allocates for CSBG.
C.3. Federal Resources Allocated (Other than CSBG) - This section is for financial accounting for all programs within your organization.
C.4. State Resources Allocated - This section encompasses all State Sector allocated resources for an agency.
C.5. Local Resources Allocated - This section encompasses all Local Sector allocated resources for an agency.
C.6. Private Sector Resources Allocated - This section encompasses all Private Sector allocated resources for an agency.
Fill In and Save Module 2 Data Entry Form
Remember, entering and saving information in the Module 2 Data Entry Form will overwrite all previously saved data. Please make sure that any information saved is relevant to your reporting period.
- Click Compliances from the system navigation bar
- Click CSBG
- Module 2 Form is shown below
- Advance through each field and enter all relevant information
- Click Save CAA Form Report when finished
Run Report for Module 2 Data Entry Form
Once your agency's information has been filled in and saved...
- Click Reports from the system navigation bar
- System Reports are shown below
- Locate Module 2 and click Run
- Set Report filters
- Date Range (required field)- Enter the reporting period Start Date and End Date
- A.1. CSBG Eligible Entity Reporting Periods:
- July 1 - June 30
- October 1 - September 30
- January 1 - December 31
- July 1 - June 30
- A.1. CSBG Eligible Entity Reporting Periods:
- All other filters are optional and can be used to make a more refined report.
- Date Range (required field)- Enter the reporting period Start Date and End Date
- Click Run Report
Print or Save Module 2 Data Entry Form
After running the Module 2 report...
- Click
Print near the top right corner of your screen - Print prompt will open
- Make any necessary adjustments under "More settings
" - Locate the desired Destination from the drop-down list
- Print - select the printer
- Save - select the save option from the list
- Click Print/Save
