Assessments Tab - The Basics
This article will focus on the Assessment Scales section. Use the links below to jump to the Assessment Packs or Assessment Collections articles.
The Assessments tab enables Agency Admins to create and manage Assessment Packs, Scales used on assessments, and Assessment Collections. These three areas are located under the Assessments tab and are broken down into three categories:
Assessment Packs - manage your organization's assessments
Assessment Scales - manage your organization's assessment scales
Assessment Collections - manage your organization's assessment collections
Assessment Scales - The Basics
Assessment scales, also called social determinants of health, are a tool with which navigators assess a client's current situation. Each scale is designed to address a specific area of client well-being while offering various outcome statements from which the client selects the most relevant to their lives. Each outcome statement is assigned a number value which correlates with an assessment scale score.
For a traditional ten-point scale, the value of ten is assigned to the most desired outcome selection (where a client is identified to be "Thriving") and the value of one is assigned to the least desired outcome selection (where a client is identified to be "In-crisis"). There are additional statement outcomes and values between the two extremes.
Traditional scale outcome statements are typically valued and scored:
- 1-2: In-Crisis
- 3-4: Vulnerable
- 5-6: Stable
- 7-8: Safe
- 9-10: Thriving
Assigning values to scales enables navigators to promptly identify and intervene when a client has been identified as "In-crisis" or "Vulnerable".
Scales can also break free from the ten-point format and use any number range, or be inverted to flip the outcome scores so the lower the assigned value is the most desirable outcome and vice-versa.

Filter Scales
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- From the Filters: drop-down lists select which:
- Domain you wish to filter displayed scales below - select "By Domain..." to reset
- Assessment Pack you wish to filter displayed scales below - select "By Assessment Pack..." to reset
- Scales matching the Filter selections will be shown below
View Scale Settings
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to view
- Click the
settings icon
Create Scale
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Click Add Scale - overlay will appear
- Enter the Scale Name
- Select the Scale Domain from the drop-down list
- Click Add Scale
- Verify the Scale Name and Domain are correct
- Select the Type from the drop-down list - learn more about Scale Types in Agency Admin - FAQ
- Enter Scale Description if desired - the Scale Description will be displayed when the assessment containing the scale is conducted in client records
- Add any applicable NPIs
- Indicate if scale is Inverted by selecting the checkbox
- Click Save Scale - if you wish to create scale options, see Create Scale Statements and Values below
Edit Scale Name
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to edit
- Click the
settings icon - Reenter the Scale Name
- Click Save Scale
Create Scale Statements and Values
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to create scale statements and values
- Click the
settings icon - Scroll down to the Scale Options section
- Type new Statement into large textbox
- Enter number Value for statement
- Click Add Option
- Option statement will appear on the right under EDIT/SORT OPTIONS
- Repeat steps 7-9 as needed
Edit Scale Statements and Values
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to edit scale statements and values
- Click the
settings icon - Scroll down to the Scale Options section
- Locate the existing option statement on the right under EDIT/SORT OPTIONS
- Click the
settings icon - Make any appropriate changes
- Click Save
Rearrange Order of Scale Option Statements
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to edit scale statements and values
- Click the
settings icon - Scroll down to the Scale Options section
- Under EDIT/SORT OPTIONS,
click and drag the Scale Options to rearrange to desired order - System will save automatically
Enable Scale "Completion Optional"
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Filter by Assessment Pack for the assessment you with to enable Completion Optional - all scales contained in the selected Assessment Pack will be shown below
- Click the
settings icon for the first scale displayed - Scroll down to the Scale Options section
- Under EDIT/SORT OPTIONS on the right, identify that there is no N/A (or other statement indicating the scale is not applicable to the client) option
- If there is an N/A option, check that the Value is set to negative one
- If Value is set to negative one, continue to step
- If Value is not set to negative one, click the
settings icon, enter -1 in the Value textbox, and click Save
- If there is an N/A option, check that the Value is set to negative one
- Under ADD NEW OPTION on the left, enter N/A (or other statement indicating the scale is not applicable to the client) in the Statement large textbox
- Enter -1 as the Value
- Click Add Option
- Scroll to the top
- Click
Assessment Scales near the top right corner - Repeat steps 4-10 for all scales contained in the Assessment Pack indicated as "Completion Optional"
View Assessments Using Scale
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to create scale statements and values
- Click the
settings icon - Scroll down to the Scale Options section
- On the right, click the Assessments using scale toggle
- All Assessments the current scale is assigned are listed - click the
view icon if you wish to view the Assessment settings
Invert Scale
Rather than the highest value be the most desired outcome selection option and the lowest value being the least desired outcome selection option, Inverted Scales flip the highest value to be the least desired outcome and the lowest value to be the most desired outcome.
Important Note: Inverted Scales and regular scales should not be used on the same assessment. If you wish to use Inverted Scales, be sure all scales on the assessment are Inverted.
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to create scale statements and values
- Click the
settings icon - Click the Invert Scale checkbox
- Click Save Scale
Add NPI to Scale
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to add a Scale NPI
- Click the
settings icon - Enter the applicable NPI in the Add NPI textbox
- Click Add
- Repeat for additional NPIs
Add NPI to Scale Outcome Statements
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the scale you wish to add scale statement NPIs
- Click the
settings icon - Scroll down to the Scale Options section
- Locate the existing option statement on the right under EDIT/SORT OPTIONS
- Click the
settings icon - Enter the applicable NPI in the Add NPI textbox
- Click Add
- Repeat for additional NPIs
- Repeat steps 7-11 for each applicable Statement Option
Delete Scale
Scales can be deleted from the Assessment Scales page or within the Assessment Scale Settings
Delete Scale via Assessment Scales Page
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the Assessment Scale you wish to delete
- Click the
delete icon
Delete Scale via Assessment Scale Settings
- Click Agency Admin from the system navigation bar
- Click Assessments
- Click the Assessment Scales tab listed on the left
- Locate the Assessment Scale you wish to delete
- Click the
settings icon - To the right of the Save Scale button, click the
delete icon
