Required Data for Care Management Clients
Case Management Client
Case Management Clients are those who are engaged in short-to-long term services and programs, with the intention of changing their status. This requires development of a goal or case plan that organizes services and strategies with clear, measurable goals and objectives with a starting and end point. Services are linked to the goal plan which has short and long term goals negotiated between the client and the case worker. Case Management clients must be reassessed at regular intervals to ensure that goals and objectives are achieved and that there is positive movement toward stabilization and/or self-sufficiency. Examples of case management clients are many and will vary from organization to organization, but might involve those receiving regular housing assistance, clients in a job training program, or those in programs for substance abuse.

Required Data for Case Management Clients:
- Section C Demographics - Collected at intake for reporting and data analysis.
- Budget/Income Verification - Collected to determine eligibility for services.
- Primary Assessment - Baseline assessment of well-being to determine clients’ needs.
- Program Entry/Tracking - Documentation that a client was assigned to an agency program(s).
- Development of a Case or Goal Plan
- Identify goals and objectives with a timetable.
- Identify achievable outcomes along the domains of well-being such as:
- Education
- Income/Financial Literacy
- Housing
- Employment
- Healthcare
- Transportation
- Provision of Service - Documentation that a service(s) was provided to the client.
- Referral Service Assignments - Documentation that a referral was made on behalf of the client.
- Reassessment - Administer the entire Primary Assessment; all scales, at the timetable established in the Case or Goal Plan.
- Employment Information - Where applicable, document employment for CSBG compliance and client case monitoring.
