Assessment History - Assessments - The Basics
Each scale on which a client has been assessed and each selected scale outcome is available via the left sidebar Assessments → Assessment History.
The Assessment History screen has four major areas:
- Assessments - this page will show all assessed scale outcomes
- Services - assign and view scale services
- Goal Plans/Projections - create scale projections and scale goal plans
- Scale Notes - create and view Scale notes
This article will focus on the Assessments area.
Each assessment scale has five outcome statuses:
- In-crisis - intervention needed, action must be taken
- Vulnerable - intervention needed, action must be taken
- Stable - reexamination if intervention is needed
- Safe - independence/self-sufficiency, intervention not needed
- Thriving - independence/self-sufficiency, intervention not needed
- N/A - the scale is not applicable to the client and is hidden by default
Regular follow-up reassessments track client movement on each individual scale and can help determine which interventions are most effective and advance client well-being.
Quick Assessment - The Basics
Please note that Quick Assessments are not intended to replace regular full reassessments. Quick Assessments are designed to document movement of a single scale. If a client experiences three or more scale movements, a full reassessment is recommended.
Quick Assessments are single-scale reassessments. They save time and eliminate the the need to conduct an entire reassessment when only a single scale needs to be reassessed. Quick Assessments can be conducted to show immediate scale movement upon service provision or the client's outcome on a single scale has changed (client was unemployed has obtained employment, for example).
Agency Admins can set a service to automatically trigger a Quick Assessment to a pre-selected outcome statement upon service assignment.
View Scale History
- Open client record
- Click
Assessments from the left sidebar
- Select "Assessment History"
- All scales on which the client has been assessed are displayed in a histogram and a list. The list shows:
- the date of the completed assessment
- the status of the selected scale outcome statement
- the scale outcome statement itself
Conduct Quick Assessment
- Open client record
- Click
Assessments from the left sidebar
- Select "Assessment History"
- Locate the scale to be reassessed
- Click Quick Assess - a screen overlay will appear
- Select the scale outcome - responses highlighted in yellow were selected in the last assessment
- Set quick assessment date - backdate the quick assessment if needed
- Click Save
- Scale has been reassessed
View Case History Report for a Single Scale
- Open client record
- Click
Assessments from the left sidebar
- Select "Assessment History"
- Locate the scale to create report
- Click
Case History Report to the right of the scale name
- Report will open in new tab
Filter Assessment Scale History
- Open client record
- Click
Assessments from the left sidebar
- Select "Assessment History"
- Use the collapsable filters on the left to filter scales by:
- Domain - display only scales belonging to the selected domain
- Scales - display only the selected scale
- Statuses - display only scales with the selected status (the most recent scale outcome selection)

Packs - display scales belonging to a specific assessment
Remember that filters can be used simultaneously which will narrow your search. Be sure to check all filters are set correctly if no results or fewer results than anticipated are shown
- Scales matching the selected filter(s) are shown
