Scale Notes - The Basics
There are three different types of case notes in eLogic Genesis. Each case note is assigned a type based on where within the system the note was created. Scale Notes are created via the left sidebar
Assessments → Assessment History.

The Assessment History screen has four major areas:
- Assessments - this page will show all assessed scale outcomes
- Services - assign and view scale services
- Goal Plans/Projections - create scale projections and scale goal plans
- Scale Notes - create and view Scale notes
This article will focus on the Scale Notes area.

View Scale Notes (via Assessment History)
- Open client record
- Click
Assessments from left sidebar - Click Assessment History
- Select the Notes toggle
- Locate the scale to which the note is attached
- Note is displayed next to histogram
Create New Scale Note
- Open client record
- Click
Assessments from left sidebar - Click Assessment History
- Select the Notes toggle
- Locate the scale under which you wish to create a note
- Click
Add Note and a screen overlay will appear - Enter the following information:
- Date Created (required)
- Hours (optional)
- Select a Privacy Group (where applicable)
- Note Tag(s) (optional)
- Note text
- Click Add Note to save
Edit Scale Note
Please note that Standard Users can only edit notes they authored while Agency Admins can edit any note, regardless who authored it.
- Open client record
- Click
Notes from left sidebar - Locate the note you wish to edit and hover your mouse over it
- Click the
more button to expand the section - Make any appropriate changes
- Click Save Changes when finished
Delete Scale Note
Please note that Standard Users can only edit notes they authored while Agency Admins can edit any note, regardless who authored it.
- Open client record
- Click
Notes from left sidebar - Locate the note you wish to edit and hover your mouse over it
- Click the
delete icon
- After the
delete icon turns into a
, that is your verification that the note has been deleted and will disappear once you refresh your browser - If you wish to undo the delete, click the
undo button.
- After the
Filter Scale Notes (via Assessment History)
- Open client record
- Click
Assessments from the left sidebar - Select "Assessment History"
- Click the Notes toggle
- Use the collapsible filters on the left to filter scales by:
- Domain - display only scales belonging to the selected domain
- Scales - display only the selected scale
- Statuses - display only scales with the selected status (the most recent scale outcome selection)
- Packs - display scales belonging to a specific assessment
Remember that filters can be used simultaneously which will narrow your search. Be sure to check all filters are set correctly if no results or fewer results than anticipated are shown
- Scales matching the selected filter(s) are shown
