Assessments - The Basics
Your organization's assessments are available via the left sidebar Assessments → Assessments. From the Assessments page, you can conduct, schedule, and view scheduled and completed assessments for every client record.
The Assessments page has four sections:
- Assessments - conduct an assessment
- Schedule Assessment - schedule an assessment for a future date and time
- Scheduled Assessments - view and conduct scheduled assessments
- Completed Assessments - view finalized assessments
You can easily jump to each section by clicking the associated segment tab:
Conducting an Assessment - The Basics
Once you begin conducting an assessment, you will be brought to the assessment itself. The assessment Scales will be listed as tabs to the left of the main screen. When a scale tab is clicked, the system will jump to that scale and outcome statements.
Advance through the assessment by clicking the appropriate scale outcome statement for each scale. Upon scale outcome selection, the system will advance you to the next scale and mark the Scale tab green, indicating an outcome selection has been made.
If you click on a scale tab that has been answered, the selected outcome statement will be indicated with green text and a checkmark.
If this is not the first assessment of its kind in the client record, the outcome statement that was selected on the most recently conducted assessment will be highlighted in yellow.
When you reach the final scale in the assessment, the Finalize Assessment section will appear below the outcome statements.
If your Agency Admin have enabled the "Completion Optional" feature, the Finalize Assessment section will always be available below the scale outcome statements. Any scales that did not have an outcome statement selected upon Finalization will be marked as N/A and the scale's value will be removed from the overall Assessment Score.

View Available Assessments
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- Click the Available Assessments drop-down list
- All assessments available to conduct will be listed

Conduct Assessment
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- Click the Available Assessments drop-down list
- Locate and click the assessment you wish to conduct
- Click Start
Finalize Assessment
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- Click the Available Assessments drop-down list
- Locate and click the assessment you wish to conduct
- Click Start
- Conduct assessment
- After the final scale outcome statement is selected, you will arrive on the Review Assessment page
- (optional) Print Completed Assessment will print scale outcome selections
- Review all scale outcome statements on this screen
- If edits are needed, click the corresponding Scale tab from the list to the left
- Select the correct scale outcome statement
- (optional) Copy assessment to family member(s) by selecting their names - this feature must be enabled by an Agency Admin to be available
- Indicate if follow-up assessment is needed by de-selecting the No follow-up needed checkbox under Next Assessment
- Select the follow-up Date and Time (optional)
- Assessment is scheduled
- Enter the Finalize Date (backdate assessments here)
- Click Finalize Assessment*
- Assessment score will be shown
- Click:
- All assessment scores to view Assessment Scoring
- Assessment History to view scales, add scale services, create a projection or goal plan, or create a scale case note

Schedule Assessment
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- Under the Schedule Assessment section:
- Click the Available Assessments drop-down list
- Locate and click the assessment you wish to schedule
- Select the Date and enter a Time (optional) for the scheduled assessment
- Click Schedule
- Scheduled Assessment will be created under the Scheduled section
- Navigators assigned to the client record will
- see the scheduled assessment on their dashboards
- receive a notification when it is time to conduct the scheduled assessment
View List of Completed Assessments
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- All completed assessments are listed under the Completed Assessment section
View List of Scheduled Assessments
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- All completed assessments are listed under the Scheduled section
- Navigators assigned to the client record will
- see the scheduled assessment on their dashboards
- receive a notification when it is time to conduct the scheduled assessment
Print Completed Assessment
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- Scroll down or use the filter tab to arrive at the Completed Assessment section
- Locate the assessment you wish to print
- Press the
Print icon next to the assessment
- In the Print Preview overlay, press "Print" - Printed assessments will show your agency's name/logo, the client's name/case number, the assessment pack name (where applicable), the selected scale/scale outcome, the name of the caseworker who finalized the assessment, the date the assessment was finalized, and signature/date fields for the client and a caseworker.
View Completed Assessment Results
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- Scroll down or use the filter tab to arrive at the Completed Assessment section
- Locate the assessment you wish to view
- Click View
- Completed assessment will open and selected scale outcomes are shown
Assessments cannot be edited after Finalization. See Assessments FAQ - "How can I edit an Assessment after it's been Finalized?" for more.
Copy Completed Assessment to Family Member(s) Record
This feature must be enabled by an Agency Admin to become available
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- Scroll down or use the filter tab to arrive at the Completed Assessment section
- Locate the assessment you wish to copy across family member(s)
- Click the
family icon
- Select Family Member(s) you wish to copy the assessment
- Click Copy Assessment
- Assessment is copied to family member record(s)
Delete Scheduled Assessment from Client Record
- Open client record
- Click
Assessments from the left sidebar
- Click Assessments
- All scheduled assessments are listed under the Scheduled section
- Locate the scheduled assessment you wish to delete
- Click the

Delete Completed Assessment from Client Record
- Open client record
- Click
Assessments from the left sidebar

Print Blank Assessment
- Open client record

FAQs
Why are some assessments not visible?
Assessments may be program-specific and require the corresponding program to be assigned.
Other reasons include:
- Insufficient role permissions (contact your Agency Administrator).

The assessment is part of an Assessment Collection—enable the Assessment Collection toggle to view it.
Why do mandatory fields appear during an assessment?
These fields are required based on Agency Admin settings and must be completed before proceeding.
How do assessments link to Module 4?
Assessment outcomes are coded to reflect NPI movements based on baseline and follow-up assessments.
How can I change the reassessment date?
Agency Admins can set the default reassessment interval under Agency Admin >> General.
Can I create custom Assessments?
Yes, custom assessments can be built under Agency Admin >> Assessments.
How are assessment outcomes scored?
Outcomes are scored from -1 to 10:
- -1 = N/A
- 0 = Null
- 1-2 = In Crisis
- 3-4 = Vulnerable
- 5-6 = Stable
- 7-8 = Safe
- 9-10 = Thriving
Can I backdate an assessment?
Yes, you can change the Finalized Assessment Date on the review screen prior to finalizing.
What appears in the Assessment History?
Scale services linked to assessments are shown, allowing caseworkers to assign services based on client status.
What is the difference between Gol Plans/Projections and Quick Goal Plans?
Quick Goal Plans are admin-defined and not linked to assessments. Goal Plans/Projections are tied to assessment outcomes and allow caseworkers to set and track programs. Completing a goal triggers an automatic Quick Assessment.
Where can I view my scale notes?
All notes, including scale notes, are found in the Notes section of the client record.
Can I edit a finalized assessment?
Finalized assessments cannot be edited. You may recreate the assessment with changes and archive the original.
What is an Auto Assessment?
An Auto Assessment is a single-scale assessment triggered by one of the following:
- The presence of a Quick Assessment on the client record
- Assignment os a service that prompts a Quick Assessment
- A family-type scale assessment conducted on a household member
